Chris, Might an Airtable, Notion or Coda form potentially work? Those tend to work well on a phone, tablet; laptop or any other device and can flow directly to wherever you need the info organized. They can also trigger automations for automatic replies or email welcome series etc

I like 3 things about Poll Everywhere for live polling: first, it has a nice text messaging function where people can reply to poll questions by text, which I tend not to use but could be useful in some circumstances. It also has some nice question types, like a 📌 pin drop question where people can point to a spot on a map or a diagram, for instance, which can be useful for asking people where they want to travel next or what part of this diagram is notable etc. and finally Poll Everywhere has a neat Keynote integration for showing poll questions or websites within a presentation. And there was helpful free teacher pricing if you have a small class up to 40 people. So I like Poll Everywhere, though I still like the simplicity and quickness of Slido.
